A 16-month-old girl, Maddy Mitchell, was smothered to death on her first day of day care in Lenoir, North Carolina, after a worker allegedly lay on top of her while scrolling on her phone to force her to nap, according to a new wrongful death lawsuit . On May 19, 2025, Alexandra Coffey, an employee at Creative Beginnings, placed Maddy face down on a mat during nap time, covered her head with a blanket, and lay on top of her torso and neck while using her legs to pin the child's legs down, the suit claims . As the toddler thrashed her free leg, Coffey allegedly held the position "for several minutes until her leg stopped moving"—all while scrolling on her phone . Coffey then got up and did not check on Maddy for three hours, by which time rigor mortis had set in and the child was dead . Coffey was arrested and is awaiting trial for involuntary manslaughter . Maddy's mother, Angel Blankenship, is seeking justice and has filed the suit against Coffey for gross negligence, vowing to fight so this never happens to another child .
